HEALTH SCREENING
  • The Health History and Examination Form should be completed and sent to the YWCA/Camp office at least four (4) weeks prior to arrival at camp. The camper will not be allowed to attend camp if the form is missing, incomplete, or has not been signed by her custodial parent/legal guardian AND a licensed physician or NP/PA. All campers must have had a physical exam within 2 years of her arrival at camp.
  • Returning the Health Form prior to arrival at camp may eliminate one part of the check-in process. All medications must be clearly labeled with the medication name, dosage, and prescribing physician’s name. All medications including over the counter medications (Tylenol, vitamins, cold remedies, etc) must be brought to the Health House to be dispensed by camp health care staff.
  • Campers must be covered by a comprehensive Health and Accident Insurance policy. Insurance information including policy numbers must be provided on the health forms you submit.
  • In the event your child requires medical attention outside of camp, every effort will be made to contact the parent/guardian to discuss options and preferences. However, our health staff’s decision to utilize the services of the camp physician or hospital will be made on a medical basis rather than on the basis of insurance coverage.
  • Our camp physician can bill Vermont Blue Cross / Blue Shield and Vermont Medicaid, for those campers needing medical care and insured under those plans. They can also bill many Vermont managed care plans if a referral has been received from your primary care provider. All other campers will have medical services billed to their store accounts. We will provide you with a copy of the bill so you may contact your insurance provider for reimbursement.
